TRAVEL INSURANCE INFORMATIONInsurance Forms and Policies
– Each member of your tour party with insurance with Worldsport must complete an insurance form.
– A parent or guardian must sign the form if the traveller is under 18 years of age
Forms once completed must be SCANNED and emailed / or / POSTED to WorldSport Pty Ltd, PO Box 411, North Tamborine, QLD 4272
Please send these registered post so they can be tracked if lost in the mail. All forms must be received at least 30 days before departure for the Carnival
